Introduction
Nurses in all professional roles work to effect positive patient outcomes and improve organizational processes. Professional nurses are leaders in problem identification, planning, and strategy implementation—skills that directly affect patient care or organizational effectiveness.
Too often, change agents jump to a conclusion that an intervention will promote the envisioned improvement. Instead, the ideal approach is to determine which interventions are appropriate, based on an assessment and review of credible evidence. Interventions could be patient-facing or involve a change in policy and process. In this assessment, you’ll identify and make the case for your practicum focus area, then explore it in depth from a leadership, collaboration, communication, change management, and policy perspective.
This assessment lays the foundation for the work that will carry you through your capstone experience and guide the practicum hours needed to complete the work in this course. In addition, it will enable you to do the following:
· Develop a problem statement for a patient, family, or population that’s relevant to your practice.
· Begin building a body of evidence that will inform your approach to your practicum.
· Focus on the influence of leadership, collaboration, communication, change management, and policy on the problem.
Preparation
In this assessment, you’ll assess the patient, family, or population health problem that will be the focus of your capstone project. Plan to spend approximately 2 hours working with a patient, family, or group of your choice to explore the problem from a leadership, collaboration, communication, change management, and policy perspective. During this time, you may also choose to consult with subject matter and industry experts about the problem (for example, directors of quality or patient safety, nurse managers/directors, physicians, and epidemiologists).
To prepare for the assessment, complete the following:
· Identify the patient, family, or group you want to work with during your practicum The patient you select can be a friend or a family member. You’ll work with this patient, family, or group throughout your capstone project, focusing on a specific health care problem.
· Begin surveying the scholarly and professional literature to establish your evidence and research base, inform your assessment, and meet scholarly expectations for supporting evidence.
In addition, you may wish to complete the following:
· Review the assessment instructions and scoring guide to ensure that you understand the work you’ll be asked to complete and how it will be assessed.
· Review the Practicum Focus Sheet: Assessment 1 [PDF], which provides guidance for conducting this portion of your practicum.

Instructions
Complete this assessment in two parts.
Part 1
Define the patient, family, or population health problem that will be the focus of your capstone project. Assess the problem from a leadership, collaboration, communication, change management, and policy perspective and establish your evidence and research base to plan, implement, and share findings related to your project.
Part 2
Connect with the patient, family, or group you’ll work with during your practicum. During this portion of your practicum, plan to spend at least 2 hours meeting with the patient, family, or group and, if desired, consulting with subject matter and industry experts of your choice. The hours you spend meeting with them should take place outside of regular work hours. Use the Practicum Focus Sheet [PDF] provided for this assessment to guide your work and interpersonal interactions. Document the time spent (your practicum hours) with these individuals or group in the Capella Academic Portal Volunteer Experience Form.

Patient under consideration and significance of the problem
The patient who needed to be examined had diabetes mellitus and hypertension. Five years ago,
the patient’s diabetes was discovered, and the proper precautions that should be taken were sug-
gested. The patient was advised on correct food management and nutritional requirements but did
not comply. Hypertension and excessive blood sugar are the outcomes. The patient was
requested to provide a follow-up periodically during the prior examination, but he did not show up
for three years. At the time of the most recent checkup, the condition was stable, but the
records for diabetes and hypertension were not enough. The patient appeared to be less con-
cerned with adhering to the instructions, but it was noticed that he still desired to be healed by the
medication. The patients’ family relatives were also present and displayed an aggressive
random history of high blood pressure. It suggested that if precautions are not taken, the patient’s
condition could worsen in the coming days. After receiving the prescribed medication, the patient
was instructed to address these issues. The patient had hypertension and diabetes mellitus, which
are significant conditions that could worsen the other health problems. The fact that 463 million in-
dividuals worldwide have diabetes in 2019 highlights the disease’s importance. It demon-
strates the need for people to be aware of potential adverse effects. In 2045, the percentage is pro-
jected to increase to 10%. Other disorders like hypertension and obesity have received alarm due to
the rising prevalence of diabetes. Regarding diseases, patients must heed the advice of medical pro-
fessionals and provide thorough follow-ups. When the family is supportive, sufferers can only pur-
sue treatment for their conditions. The issue is pertinent to my nursing practice since diabetes and
hypertension are widespread yet complex diseases. The nurses are responsible for ensuring
the patients are adequately advised and motivated to take their medication. These are the prob-
lems that can be controlled but are not sufficiently resolved. A record of diabetes and hypertension
should be kept during the follow-up process. However, it has been shown that patients frequently
misinterpret the instructions and disregard them. Future research on managing diabetes and hy-
pertension patients will benefit from this knowledge. I’ll discover how to communicate with patients
and properly guide them. Analyze evidence from peer-reviewed literature and professional

suggested that nurses follow specific procedures for managing hypertension in diabetic patients. A
blood pressure of 130/80 mmHg for patients with both illnesses is advised. The writers have sug-
gested the goal based on a substantial amount of scientific research. Less research has been done
on the advantages of blood pressure medication for diabetic people. If the blood pressure sur-
passes 140 mm/Hg, the American Diabetes Association (ADA) advises that it will be a significant
cause for concern. Proper care and management are required for the patients. The results of nu-
merous research have also been reported (Kenny, 2018), which show that diabetes mellitus and hy-
pertension are important issues that need to be handled. Salt and sodium should be consumed in
smaller amounts. It is advised that patients eat a balanced diet of fruits and vegetables and engage
in regular physical activity to maintain a healthy lifestyle. In certain papers, the statistics are dubi-
ous, yet the nurses must decide whether or not they are real. The peer-reviewed articles and their
techniques will be utilized to evaluate the data’s dependability. It is important to note that erro-

neous data is not regarded as authentic and significant. The difficulties in implementing evidence-
based practice have been addressed (Lynne S. Robins, 2022). Patients and their families occasional-
ly lack the motivation or assistance to adhere to diabetes and hypertension standards. It has been
noted that while social and cultural norms are significant enough to be considered during the thera-
py process, they also function as a barrier. To effectively manage diabetes mellitus and hyperten-
sion, impediments must be kept to a minimum. The efficacy of nursing standards and policies
in enhancing the health of patients with diabetes and hypertension has been studied (Cheryl R.
Dennison Himmelfarb, 2017). It is advised that nurses play a crucial role in the healthcare in-
dustry. To effectively treat diabetes or hypertension, their duties must be enlarged. For the benefit
of the patients, their duties need to be increased. The management of these illnesses is subject to
criticism for several roles and laws. The different disorders must be found and identified by the
nurses.

They must communicate with the patients and help them understand their problems. According to
recent literature, nurses should identify patients’ ideas and social norms to determine how
they should be handled. The nurses must ensure that the patient’s diseases are treated according
to the family’s perspectives after researching them. There is considerable literature on research
connected to diabetes and hypertension. However, this topic is about the management role of nurs-

policies could affect the patient Numerous nursing practice guidelines or governmental regulations
exist for patients with diabetes and hypertension. The nursing policies show they are sufficient to
handle these patients, but they require improvement. For the benefit of the patients, several official
documents are published and displayed, and the function of the nurses is also described.
According to the 2019 American Heart Association (AHA) Scientific Statement on measuring BP in
Humans, nurses should retrain every six to twelve months. The rule stipulated that nurses must
possess the necessary qualifications (Melnyk, 2022). The regulations are created at the state level,
and the nurses responsible for managing diabetes and hypertension are given several instructions.
According to WHO guidelines, nurses should adequately identify patients with diabetes and hyper-
tension. The nurses must intervene if the patient consistently displays a hypertension history
and blood pressure higher than 140 mm/Hg. The World Health Organization and the American
Nurses Association have served as front-line combatants in the fight against numerous diseases
and have provided nurses with specialized regulations. The nurses care for the patients by adminis-
tering the proper medication and interacting with them. The group has provided specific recom-
mendations for treating hypertension. The organization instructed the nurses to be empathet-
ic and watchful in treating the patients since it is concerned about other ailments, including cardio-

A vital component of the healthcare industry, nurses must engage with patients and provide guid-
ance. Through exemplary leadership and change management techniques, the nurses can bet-
ter advise the patients. The nurses must be aware of the cooperative and communication tactics
and implement them. Nurse leaders must adhere to four ethical guidelines. These include au-
tonomy, social justice, kindness, and informed consent. Nurses must guide and demonstrate sym-
pathy for patients to heal by following the correct instructions. The leadership tactics nurses use in

relation to the topic being studied through several research studies. In these urgent situations, the
leaders are advised to obtain the patient’s informed consent (Paarima, 2022). Collaboration is
essential for patient improvement, and communication is the key to success. Patients won’t be
able to understand the treatment process if the nurses don’t communicate well. Let’s say the
nurses believe the situation is not improving and the patients or junior nurses are not cooperating.
Change management must be put into practice to further improve the results in that situation.
Conclusion It has been determined that nurses must manage a variety of ailments and diseases.
Throughout the treatment, the nurses must retain both compassion and discipline. Disorders
like diabetes and hypertension must be well maintained. The nurses are responsible for ensuring
that the ethical standards are followed and that the leadership standards are met.
